Post-operative soft tissue management

Key points

  • The postoperative soft tissue management can coincide with the regular control recalls.
  • The patient is seen one to two weeks after dental implant placement for suture removal, postsurgical instructions, and observation of healing. In case of one-stage or immediate loading protocols, soft diet is recommended for 30 days.
  • The patients are instructed to rinse the mouth with 0.2% chlorhexidine mouthwash thrice a day without brushing the implant area until suture removal (7 to 14 days after).
  • Evaluate wound healing and manage any signs of inflammation or infection.
  • Patients would greatly benefit by having dental implants placed flapless, reducing the postoperative pain and consumption of analgesics.

In many instances, single-tooth implants provide exceptionally esthetic restorations but soft tissue deficiencies can detract from the esthetic result. Although some mucosal changes may not be noticeable to patients, others may produce significant esthetic deficits in highly visible locations of the mouth. For example, loss or reduction of interdental papillae leads to open cervical embrasures with resulting dark interdental spaces which also are potential sites for food entrapment.

In a contemporary dental practice implant dentistry should be recognized as an accepted treatment modality based on decades of success. Consequently, clinicians who provide implant surgical and/or prosthodontic treatment should develop and routinely provide patients with a dental implant oral hygiene maintenance protocol to ensure the longevity of the treatment provided. In addition, clinic staff and implant patients themselves should be advised that research has shown maintaining peri-implant tissue health is a key factor related to the long-term survival of dental implants. Furthermore, the maintenance procedures for a dental implant patient will differ in some respects from the program provided for fully dentate patents.
