In case of complex and extensive rehabiitaitons is necessary to proceed with a complete data collection that includes X-Ray,Intra and Extra Oral Pics, study models and CBCT.
The anodization and the smear layer thickness increase in the apex area? if does, why don't we have a colour change?
The change in color of Ti surface is not proportionally correlated to the surface roughness. The Titanium di-oxide layer thickness in the apex area is really big, and it considers a micro and nano geometry.

I think it is interesting that we are retrofitting bone level implants...
and we are doing this with more components. There are implant brands with different transmucosal machined distances. This is only a way to retrofit an Active implant to get better bone rather than reworking the implant.
If you place a transmucosal implant where you want it and place a screw retained fully contoured restoration, you have achieved the same thing without further possible expense, complexity and components to deal with.
This is a solution to a problem created by the marketplace. Bone level implants are not ideal in all locations but they are heavily promoted. Basically we are going back in time with more complexity and I don't think this is the best for patients in the long run.

Bone level implants combined to final intermediate abutments means to create a customized tissue level concept in each clinical situation independently from anatomical conditions such as vertical soft tissue thickness.
REVERSABILITY AND FLEXIBILITY are other important advantages.

Yes, you want it to come into contact with blood. Slow insertion speed ensures that the friction during insertion does not overheat and damage the tissue so lack of irrigation should not be an issue.

Yes, I keep the surface pristine, and I don't use irrigation.
