The rehabilitation of severely atrophic mandible using implant‐supported prosthesis is often challenging because of the poor quality and quantity of residual jawbone, especially in patients with long‐term edentulism. The combined use of axially placed and tilted implants are an alternative to rehabilitate fully edentulous jaws without resorting to bone grafting. Follow the detailed surgical steps of this treatment approach applying a conventional flap procedure with traditional planning and surgery using a standardized All-on-4® Guide.

I guess it is because of the lack of bone height on the posterior region.
This is a restoration with full-arch suprastructure, then I guess they needed to extract the front teeth to place the implants.
