Ken Parrish: Minimizing complications with full-arch immediate provisionalization

What can go wrong when restoring a full-arch with immediate provisionalization ? Dr Parrish gives insight in the most common complications and pitfalls and how they can be avoided. Often failures are due to human errror. Implant component failures are rare, provided premium quality products are being used. Most common post-op complications following full arch provisionalization are fracture of the all-acrylic restoration and teeth, and screw loosening. Dr Parrish discusses, that screw loosening is due to misfit of the restoration and can be avoided e.g. by direct pick-up of the components in the mouth rather than using an indirect protocol with impression and model. To avoid fractures, the provisonal restoration needs to have sufficient dimensions and/or other reinforcement. Other reasons for restoration fracture can be loss of support to to inadequate seating of abutment or failure of implant and occlusal forces/parafunctions.
