It has been stated that the periodontist's primary role is to maintain the natural dentition in health, function and esthetics. Patients' dentitions can fail for a variety of reasons, periodontal bone loss being only one of them. As periodontal treatment concepts of disease and regeneration have expanded to incorporate implants, fixed options may be the best solution for fulfilling treatment goals. The All-on-4® treatment concept however, is to some a paradox; removal of alveolar bone may improve the health, function and esthetics of fixed tooth replacements. The lecture will provide a greater understanding of bone reduction and how improvements in esthetics, function and health can be achieved through it.
