The lecture covers general considerations of the All-on4 concept and aspects of biomechanics, restoration design, space considerations, materials used, occlusal concepts, maintenance and more. The All-on-4® concept can be considered as a graftless solution, and may apply also to treatment plans comprising 5 or 6 implants. Long cantilevers can significantly magnify forces and might have a detrimental effect, especially in immediate loading situations, therefore cantilevers should be as short as possible. Restorative considerations comprise incisal edge position, restorative space, lip support, smile line and lip length, contours and emergence, appropriate tissue contact and occlusal contacts. Depending on the restorative space the prosthetic design options range from a PFM/PFZ bridge solution over a profile prosthesis to an overdenture with flange to provide adequate lip support.
